For self-managed and plan-managed participants
We support you as you go about your daily living activities. Personal care refers to support with personal hygiene, grooming, dressing and other essential activities for maintaining your physical health and wellbeing. Our friendly and experienced team can help you with all these tasks, enabling you to maintain your independence, improve your overall health, and increase your confidence and self-esteem.
We support you with the day-to-day tasks that keep your home clean, safe, and comfortable. Household Tasks include a range of activities such as cleaning, tidying up and laundry. Your friendly and experienced support team can assist you with all of the above, as well as meal prep and basic home maintenance.
Our aim is to help you maintain a clean and comfortable home environment, so that you can live life as independently as possible and enjoy your surroundings.
Reliable transport when and where you need it can help you feel more independent and connected to your community. Whether it's a trip to the shops, a medical appointment, or a social outing with friends, we're here to provide accessible transport wherever you need to go.
We can also help you build your skills in navigating the public transport system, so that you can maintain your independence and access all that life has to offer beyond your front door.
Community Participation is all about helping you get involved and connected to your local community. This NDIS support category covers a range of activities, such as attending social events, joining a sports team, a local program or volunteering.
Our friendly team is well-connected with local community groups throughout the North Shore, meaning that whatever your intersts might be, we have programs and activities to suit you. Our goal is to help you feel more confident, engaged and connected to the people and places around you.
Community Nursing focuses on providing clinical care and support for people with complex health needs. Our highly experienced nurses support you with a range of health-related tasks, such as medication management, wound care, and clinical assessments. We can also help you to manage chronic health conditions.
We work closely with other healthcare providers to ensure that your care is coordinated and tailored to your individual needs. Whether you require a short drop-in visit or ongoing support, our nurses can provide the best of clinical care in your own home.
Our support starts with empathy and compassion
Together, we build a support plan that works for you - both in terms of your daily needs and your long-term goals.
We collaborate regularly with you and your family to make sure our supports are helping you to achieve your objectives.
As you needs evolve over time, so too do our services. We believe that flexibility is key to providing the best possible support.
